The personal needs of our residents are unique to the individual. That's why our Social Services Director provides full-time assistance in scheduling outside appointments, placing phone calls and arranging for home care and community services as part of our discharge planning program. We review the discharge plan periodically. Should a resident improve to the point of needing less skilled attention, we can recommend programs of alternative care including referral to a personal care facility, or home nursing.
